About the Role

As the Clinical Manager, you'll oversee the delivery of high-quality, patient-centered care in a home setting. This dynamic leadership role blends clinical expertise, operational oversight, and compassionate coordination to ensure our patients receive the care they need—when and where they need it most. You'll mentor a team of skilled nurses, therapists, and aides, while acting as the bridge between clinical staff, patients, families, and administration.

Key Responsibilities

  • Team Leadership: Hire, train, supervise, and support a multidisciplinary team. Conduct performance evaluations and foster a culture of excellence and accountability.
  • Patient-Centered Care: Oversee and guide individualized care plans in collaboration with physicians and other care partners to ensure timely, effective interventions.
  • Regulatory Compliance: Ensure adherence to all federal, state, and agency regulations, including Medicare/Medicaid guidelines and Joint Commission standards.
  • Quality & Performance Improvement: Conduct audits, monitor outcomes, and implement strategies to continuously elevate the quality of care.
  • Advocacy & Support: Serve as a trusted point of contact for patients and families, addressing concerns with empathy and efficiency.
  • Clinical Documentation: Review and approve documentation for accuracy, completeness, and compliance.
  • Budget & Resource Management: Assist in budget oversight and resource allocation while maintaining high standards of care.
  • Staff Development: Deliver ongoing education on clinical best practices, regulatory changes, and internal protocols.
  • Cross-Functional Collaboration: Partner with administrative and operational teams to drive care coordination and patient satisfaction.

Requirements

What You’ll Bring

  • Education: Bachelor of Science in Nursing (BSN) required; Master’s in Nursing or Healthcare Administration preferred.
  • Licensure: Current RN license in the state of practice.
  • Experience: 2–3 years in home health or a related clinical setting, with proven supervisory or management experience.
  • Skills:
    • Inspirational leadership and team development
    • Strong interpersonal and communication skills
    • Solid understanding of home health regulations and reimbursement (Medicare, Medicaid)
    • Proficiency with electronic health records (EHR) and clinical documentation
    • Critical thinking and sound decision-making under pressure
  • Certifications: BLS required; Home Health Care Certification preferred
